The original short story was published in Collier’s magazine dated January 11, 1947 – here are page images from the marvelous site [You must be registered and logged in to see this link.] It may have been published earlier elsewhere, or submitted to Suspense as a soon-to-be-published story in 1945 or 1946. The original performance was in February, 1946.
Note that the title was Revenge!

The site also has the contract for the 1946 performance. Stewart was paid $4000 == that’s $49,800 in today’s dollars.

Consequence was presented twice, but the short story it was based on was titled Revenge.
Nightmare is a different story.

Ah yes, in the Indianapolis paper! -- making a note in the post as well. Great catch!
Also note that the plot of this is different from a (one-hour) performance of the same name done on March 13, 1948 with Eddie Bracken.

This is an interesting one -- the story as a concept was sold to a studio, with the writers contracted to produce a movie script. Along the way to creating that treatment, they turned their idea into a Suspense script. This is not another Suspense radio play that became a movie, because the script predated a sold movie treatment of the original idea. Or it would have... but it seems a movie was never made.
